This proven charting solution is implemented in over 500 US hospitals. T System for Nurses includes 28 comprehensive templates covering virtually all presenting complaints in a variety of categories such as medicine, trauma, pediatrics, OB/Gyn and more.
Designed by clinicians, T-System templates are based on a thorough understanding of the unique needs of ED nurses. Legible documentation is easy with the use of simple circles and backslashes that allow nurses to clearly and quickly record positive and negative findings, greatly reducing the need for free text. Standardized formatting and organization supports APC capture while addressing JCAHO requirements and other compliance issues.
T-System provides nurse clients with shelving units that offer convenient access to their templates. The shelving unit has two columns with color-coded labels that indicate whether the templates pertain to Trauma, Add-On or Medicine. Clearly categorized by chief complaint, nurses can quickly pull the appropriate chart and immediately begin documentation. Shelving units should be placed in easy-to-reach areas throughout the emergency department for optimal organization and workflow.

| Threshold Set Option |
T-System's Threshold Set uniquely encompasses all of the trusted components of the original T System for Nurses but with additional features that improve nurse accuracy by eliminating the need to count points for code leveling and compliance. Based on ACEP's leveling threshold system, T-System's Threshold Set streamlines workflow and contributes to overall nurse efficiency.
Threshold numbers are embedded into each template and instantly show the accurate E&M coding level for a particular patient visit. In the templates, nurses will see numbers 1 through 5 - or "cc" for critical carenext to targeted elements of the medical record. Once charting is complete, the nurse simply reviews the patient template and locates the highest number, which indicates the appropriate coding level.
T-System also provides nurses using the T System Threshold Set with shelving units as previously described, offering convenient access to the templates.
