Care Continuity, a component of the T-System Performance solution, is an integrated web-based referral management tool that connects the care team to a patient's information, allowing for a virtual way to build and maintain relationships with a much broader group of referring physicians.
How it Works
T-System Performance Care Continuity automates workflows to ensure that the patient-handoff process is smooth and seamless:
- Invite any physician to connect to receive referrals and access the patient record.
- Participating physicians receive immediate alerts when their patients register in the emergency department (ED), are admitted or discharged.
- If the patient's primary care physician (PCP) is not currently participating, the patient's record is sent to the work queue for follow-up with the physician liaison.
- If the patient does not have a PCP, a participating physician is assigned.
- The records of patients at risk for readmission are sent to the work queue for follow-up with case management.
