Care Continuity, a component of the T-System Performance solution, is an integrated web-based referral management tool that connects the care team to a patient's information, allowing for a virtual way to build and maintain relationships with a much broader group of referring physicians.

How it Works

T-System Performance Care Continuity automates workflows to ensure that the patient-handoff process is smooth and seamless:

  1. Invite any physician to connect to receive referrals and access the patient record.
  2. Participating physicians receive immediate alerts when their patients register in the emergency department (ED), are admitted or discharged.
  3. If the patient's primary care physician (PCP) is not currently participating, the patient's record is sent to the work queue for follow-up with the physician liaison.
  4. If the patient does not have a PCP, a participating physician is assigned.
  5. The records of patients at risk for readmission are sent to the work queue for follow-up with case management.
