T-System's Solutions Reduce Patient Readmissions and Meet Meaningful Use at HIMSS13

DALLAS – Feb. 25, 2013 – More and more hospitals working to embrace value-based reimbursement and regulatory reform while tackling complex community health needs are focused on how the emergency department (ED) can help support these goals. To prepare them for two immediate challenges, T-System Inc. will demonstrate its high-tech solutions and strategies proven to reduce readmissions and complete Meaningful Use requirements at the Healthcare Information and Management Systems Society's (HIMSS) annual conference and exhibition, March 3-7, at New Orleans' Ernest N. Morial Convention Center.

At its exhibit booth No. 2219, T-System will present PerformNext™ Care Continuity, advanced web-based technology that actively manages patient care transitions to reduce avoidable readmissions. The solution will help hospitals now held accountable by The Centers for Medicare and Medicaid Services (CMS) avoid stiff penalties deducted from their Medicare reimbursements if patients are readmitted within a month of discharge.

At the first HIMSS Meaningful Use Experience, a new dedicated area showcasing certified electronic health record (EHR) systems, attendees can view EV™, top ranked in this year's KLAS ED information system (EDIS) report for physician adoption, improved charting and documentation accuracy and improved throughput. EV is widely recognized as one of the most successful EHRs helping support hospitals' enterprise strategies achieving Stage 1 thresholds.   • Cox Medical Center Branson is aimed at reducing avoidable readmissions by 20 percent using Care Continuity in its hospital and 19 clinics. By proactively identifying high-risk readmission patients and connecting them with the appropriate resources for aftercare, the facility hopes to avoid any reductions in Medicare reimbursement for exceeding 30-day readmission levels.

  • Baptist Health of Kentucky achieved every large health system's dream: Five implementations in six months, on schedule, under budget, with near 100 percent adoption. Within 24 hours of activating the EV, 90 percent of the Meaningful Use computerized provider order entry's (CPOE) requirement was achieved, exceeding threshold metrics the first day of go live.

EV's capabilities are highlighted in the e-session #407 titled "An Experienced RN's Seven Commandments for Healthcare Project Management Success" delivered by Patti Hope, MHI, BSN, RN, lead clinical business analyst, at Baptist Health of Kentucky. The e-session is being shown 8 a.m. to 5 p.m. Central March 4-7 in two conference center locations: Lobby G (first floor) and outside of room 260 (second floor). A 30-year nursing veteran with 17 years' experience directing clinical IT initiatives, Hope spearheaded the EV with CPOE implementation in five facility EDs within six months.
