Care ContinuityWhen I joined T-System nine years ago, the healthcare IT industry was buzzing with speculation about electronic health record systems. Today, in the midst of the current electronic healthcare environment we once only dreamed of, the industry is again changing. Our focus is evolving from making patient information more accessible to providers to making information more actionable.

As T-System's vice president of T-System Care Continuity, my mission is to help the unscheduled care market embrace this new action-focused approach to care delivery. T-System Care Continuity will help reduce avoidable readmissions, improve patient outcomes and ultimately retain patients in the network.

Recently I've been working with Cox Medical Center Branson, a 165-bed hospital in southwestern Missouri, to tackle its readmissions rate. The facility is using T-System Care Continuity to achieve an aggressive 20 percent reduction in readmissions. The solution instantly gives community physicians access to information about patients who have been admitted to or discharged from the Cox Medical Center ED. But Care Continuity also automates the patient transition workflows, which ensures patients receive appropriate follow-up care within the network. By tracking the patient to his or her next care setting, Cox Medical Center has eliminated the possibility of anyone falling through the cracks of the system. This has led to improved patient outcomes and reduced cost.

This is an exciting time for our industry and for T-System, and I'm looking forward to showing you how the ED can play a pivotal role in the transformation to an actionable healthcare system.
